Following documents must be submitted / attached with the application form:
IN CASE OF PERSONAL APPEARANCE (CATEGORY A OR B)
1. Hand written application with Registration No., current address. Download 2. Original Category 3. Original File (if applicable) 4. Pay Order / Demand Draft of Renewal Fee in the name of Secretary, Punjab Pharmacy Council of Rs. 5000/- (thousand per year) .
IN CASE OF NON-APPEARANCE (CATEGORY A OR B)
1. Hand written application with Registration No., current address. Download 2. Two (2) photocopies of Category 3. One (1) Fresh Photograph 4. Original File (if applicable) 5. Appearance certificate issued by Area Drug Inspector containing name & details of the Area Drug Inspector. 6. Should provide valid Biometric receipt. 7. Pay Order / Demand Draft of Renewal Fee in the name of Secretary, Punjab Pharmacy Council of Rs. 5000/- (thousand per year).
IN CASE OF LIVING ABROAD (CATEGORY A OR B)
1. One (1) colored photocopies of Registration Certificate Copy (A or B). 2. One (1) colored photocopies of VISA. 3. One (1) colored photocopies of passport. 4. One (1) colored photocopies of CNIC. 5. Hand written application with Registration No. with current Address Download 6. Authority letter. 7. Pay Order / Demand Draft of Renewal Fee in the name of Secretary, Punjab Pharmacy Council of Rs. 5000/- (thousand per year) . 8. The above required Scanned Data along with request shall be sent on the email address of Punjab Pharmacy Council i.e. (punjabpharmacycouncil@hotmail.com)
