Duplicate Registrations A & B (Change of Name)

Requirements of Duplicate Category A or B (Change of Name)

 

1. Application for Duplicate  Registration with reasons (Change of Name)

2. Two (2) colored photocopies of CNIC

3. Three (3) specimen signatures duly attested

4. Four (4) Photograph Passport size

5. Four (4) Color photocopies of degrees of B. Pharmacy, Pharm – D reflecting the change of name 

6. Fee for Degree verification as prescribed / required by the University Concerned.

7. (Degree Verification fee Challan concerned university)

8. Pay Order / Demand Draft for issuance of Duplicate Certificate Fee in the name of Secretary, Punjab Pharmacy Council of Rs. 5000/- .

9. Surrender of original Registration Certificate

10. Personal Appearance in Punjab Pharmacy Council / Appearance certificate from Area Drug Inspector (if necessary).

       All required documents attested by the concerned University.
